The data provided is intended solely for registration with the Iceshow Associação Freestyle and registration as a federated athlete with the Portuguese Skating Federation, also including the data necessary to take out the annual sports insurance provided by this Federation, which are only valid until December of the current year.

To be able to access the payment of monthly usage fees, you must subscribe to this registration.

This registration is effective from the date of its validation until the end of December of this year. Registration only becomes effective after valid collection of the respective amounts.

This form constitutes an application for registration and does not imply acceptance. The association reserves the right not to accept applications. Confirm later that your application has been accepted.

IMPORTANT NOTES:

Said registrations only become effective and are the responsibility of Iceshow Associação Freestyle after the following conditions have been met:
1) Payment can only be made ONLINE by bank card, PayPal, or Google Pay.
2) Carrying out and submitting a sports medical examination within 30 days after the date of registration.
3) Sending the federative registration form after it has been validated and signed, which will imply the activation of the insurance.
4) The registration of skaters at the Iceshow Associação Freestyle school, as well as in any other sports association, requires the subscription of sports insurance until January of the following year. Request information from those responsible for the skating school. The subscriber or his/her responsible person must accept that the insurance subscription is only activated after sending to the FPP and closing the respective Registration Guide with all signed documentation (FPP Registration, Declaration of Consent and Medical examination approved by the responsible doctor.
